Our company standards and expectations, all of which were included in your initial job description for the position, were discussed and reviewed.

You were given a copy of your most recent performance evaluation which rated your overall job performance as unsatisfactory. It was greed by both of us, that the area requiring immediate improvement was in the quality control portion of your job responsibilities. As a solution, you agreed to adhere to the stringent quality control standards as outlined in the production manual. We will monitor your progress for a month, after which you will receive another performance evaluation.
